Young Life Counseling Benefits

For those enrolled in either of the Young Life medical plans, two separate counseling benefits are offered to you and your family members. As this can sometimes be confusing, listed below is an explanation of each benefit and how claims are submitted to UMR for payment.

Standard Mental Health Counseling

These services are typically billed with a diagnosis code starting with the letter “F”, and cover counseling services for conditions such as anxiety, depression and adjustment disorders. These are paid like any other office visit at the in or out-of-network rate once your annual deductible is met. There is no dollar limit on these services or a maximum on the number of visits. If you see an in-network counselor, that provider should bill UMR on your behalf.  However, if you’re seeing an out-of-network counselor, you usually have to pay for the services as you go, and then submit the bills directly to UMR. The counselor should provide you with a “super” bill that includes his or her credentials and all the insurance codes. Please use the 1708 UMR claim form to submit these services for a reimbursement.

Marriage, Family and Individual Counseling (MFI)

This is a special benefit covering counseling services that we refer to as “family of origin” such as marriage counseling, counseling for parent/child issues, upbringing issues — anything that relates to family relations. The benefit is $4,000 per family each calendar year and is reimbursed at 100%. Please note: if you are enrolled in the 80/50 High Deductible Health Plan, these services will apply to the annual deductible before reimbursement is available. The counselor must be licensed and provide you with an itemized bill that lists a covered diagnosis code or written designation that relates to the particular family issue you’re seeking treatment for — all of these are outlined on the 1706 claim form. For example, marriage counseling (Z63.0) or family of origin/upbringing (Z62, Z62.9). You will need to submit your invoice(s) directly to UMR along with form 1706 each time you are requesting a reimbursement. The counselor also has the option to sign the form and check one of the appropriate boxes if he or she does not wish to put one of the covered codes on the invoice. You may review the full benefit here.

Additional Information

The email address for UMR is listed on the bottom of both claim forms. And UMR now offers the option to have your reimbursements directly deposited into your bank account. You may enroll in direct deposit by logging into your account at www.umr.com, hovering over “Consumer Accounts” and then clicking the “Direct Deposit Authorization.” If you don’t have this option activated, you will be sent a check in the mail to the address listed on your Workday account.  Services must be submitted within 12 months of the incurred dates, and you may submit multiple billings at a time with one claim form.
